Two licensing coordinators review of program documents revealed that there was not an allergy action plan on file for a child with a severe allergy requiring epinephrine. Program staff confirmed that they had not obtained this documentation from the child's family. Corrective action: Site director has been in contact with the family to obtain the allergy action plan to have on file. Anticipated date of receipt by 11/8/2024 Moving forward, action plans will be on file before members can start attending programming.,
Two licensing coordinator's review of three staff files revealed that one staff member who had been employed longer than 90 days had not completed the required health and safety trainings. Corrective action: Staff member in question is required to complete trainings prior to being placed in program. As of 10/31/2024, staff has completed 9 out 10 of the required health and safety trainings. Expected completion by 11/8/2024. Moving forward, all staff files will be reviewed monthly to ensure compliance.
Two licensing coordinator's review of nine children's files revealed that one did not contain written parental permission for emergency medical transportation and treatment, signed by the parent upon the child's first day in attendance in the program. Corrective action: As of 10/23/2024, the program has received signed permission for emergency transportation and treatment. Moving forward, all members files will be reviewed monthly to ensure compliance.
Two licensing coordinators review of three staff files revealed that one staff who had been employed for longer than 90 days did not have documentation of CPR and First Aid certification. Corrective action: Staff is currently enrolled in an EMT course which will cover CPR certification. Staff member is determining date of CPR course. If course is not in near future, Site director will send them to a nearby CPR course. Moving forward, all staff will have a CPR training scheduled within their first 90 days of hire.
Two licensing coordinators observed one epinephrine injector and one albuterol inhaler for which there was no parental authorization on file. Corrective action: As of 10/30/2021, the program has received Authorization form for the albuterol inhaler and epinephrine injector. Moving forward, staff will not accept emergency medications without a parental authorization on file.
Two licensing coordinators observed one epinephrine injector that did not have a prescription label or medication order from a licensed professional. Corrective action: Site director has been in contact with the family to obtain the prescription label for the epinephrine injector. Anticipated date of receipt by 11/8/2024 Moving forward, staff will not accept emergency medications without a doctors prescription on file and a label on each medication.
The licensing coordinator's review of the program's fire drill log revealed that fire drills were not conducted in each month of operation. Staff members confirmed that they had missed several fire drills during the previous year. Corrective action: We have put fire drills into digital and physical calendars for specific times and days each month, with reminders occurring a week in advance and day of. Fire Drills for October and November have taken place.
