Programs shall comply with approved corrective action plans and corrective action plans issued in accordance with He-C 4002.06(i) and (j).

This Statement of Findings includes a repeat citation and failure to comply with an approved Corrective Action Plan for He-C 4002.41(a), as noted herein. Corrective action: We will monitor quarterly all staff and household member background checks expiration dates and ensure they are renewed in a timely manner. While reviewing the household and personnel form we became aware that another household member has just turned ten years of age and must be have a form completed. We will be submitting a form for that member immediately.

Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

REPEAT CITATION, PREVIOUSLY CITED ON MARCH 18, 2022. The licensing coordinator's review of department records revealed two household members did not renew their background record checks prior to their 5 year expiration dates of November 28, 2023 and November 30, 2023. Additionally, one household member did not submit for a background record check utilizing the NH Connections Information System as required. Corrective action: Upon review we determined that the household members' background check never cleared NHCIS because the household member did not understand they needed to complete additional steps through the online program. The background check continued to list as pending. The household member has taken the additional steps to successfully complete the process. We will continue to monitor all employee and household member expiration dates and ensure that everyone is renewed in a timely manner. All household members will not be down in the program or interacting with children until deemed eligible. All household members are in the process of getting the BRCs taken care of.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The licensing coordinator's review of eight child files revealed that none of the children's registration and emergency information forms were the equivalent of the April 2022 form, as the forms were missing the required statement under He-C 4002.10(c), and the forms on file had not been updated annually. Corrective action: I immediately destroyed the old forms that were being used and made copies of the correct form to be used in the future. Our last day of school is Tuesday so we will not be able to change them for this past school year. We are just now taking registration information for the school year 2024-2025 and will be sure that the correct emergency form is used in the the Fall and moving forward.
