The program shall request and maintain on file for each child documentation of immunizations in accordance with RSA 141-C:20-a, RSA 141-C:20-b, and He-P 301.14.

The licensing coordinator’s review of 12 child files revealed that there was no record of immunizations on file for one child, when required on the child’s first day of attendance in the program. Additionally, there was no record of health assessment on file for one child, when required within 60 days of the first date of attendance. Corrective action: Required health documents will be obtained and filed within the appropriate child files on site. Parents will be notified at the time of registration that their child cannot attend program until the required health documents are submitted.

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

The licensing coordinator’s review of three staff files revealed that two staff had zero hours of professional development in 2024, when 18 hours of professional development are required annually. Corrective action: We will comply with this by creating and maintaining a professional development spreadsheet for staff. Staff will be enrolled and complete the required number of professional development hours each year.
