The complainant stated that when Staff A was transporting a child to school, it was unclear who was caring for the children remaining in the residence. Staff A confirmed that they transported a child to school on two days, and that they left a household member alone to supervise the other children for up to 15 minutes in each instance. The licensing coordinator's review of the household and personnel list revealed that the household member did not submit for a background record check upon returning to the home as a resident. Corrective action: My daughter turned 18 a year ago and has moved in and out a few times. as of last week, April 2&3, her BRC and fingerprinting have been done I understand that leaving the children with her was against licensing rules, its important to note that all the parents were aware that my daughter sat with the children and they were all okay with it in the future i will set reminders to insure if and when my daughter moves in and out again that her brc stays current

The licensing coordinator's review of attendance records revealed that none of the three children in attendance were signed into the program on the day of the inspection. The provider reported they had not completed attendance records for the week. Corrective action: hung clip board from bookcase in direct site, making it harder to forget 9/2 Attendance records will be completed as children come and go each day.

The licensing coordinator observed a tube of super glue on the kitchen counter which was accessible to children at the program. The glue was immediately removed from the drawer and placed into a high cabinet during the inspection. Corrective action: yes licensing staff moved it Later I returned it to its rightful place in my bedroom where children are not permitted 8/28

Except for nature-based programs that operate solely outside, programs shall conduct fire drills in accordance with the following: (1) Programs that operate continuously throughout the year shall conduct at least one drill each month of operation; (2) Programs that operate only during the school year shall conduct a drill within 14 days of opening each year and monthly thereafter; (3) Programs that operate only during the summer months shall conduct one fire drill in each month of operation; (4) Child care personnel shall conduct fire drills at varying times during operating hours, including night time hours, if applicable, to ensure that each child attending the program experiences fire drills; (5) Child care staff shall activate the actual fire alarm system for the building for at least 2 of the required monthly fire drills required each year and use a fire alarm or smoke detector to signal all other fire drills; (6) All children and child care staff shall evacuate the building during each fire drill; and (7) Child care staff shall check daily attendance records to ensure that all children and staff are accounted for, after the building is evacuated.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.

The licensing coordinator’s review of six children’s records revealed the child care registration and emergency information form was missing in one of six files reviewed and had not been updated annually in four of the six records reviewed. Corrective action: had parents sign confirming no change in info 8/29 the one missing is coming this Wednesday 9/25 The first of each month i will do a file check and have parents update and sign their registration information as needed to keep all information up to date.
