The licensing coordinator's review of 10 child files revealed that seven of the Child Registration and Emergency Information forms had not been updated annually. Corrective action: All parents have completed emergency info forms. We typically have all parents update the forms in January each year with the assistant director however she was on maternity leave in January. In the future, we will have all parents fill out a new emergency info form in January of each year to ensure all forms are up to date.
REPEAT CITATON PREVIOUSLY CITED APRIL 4, 2022 AND OCTOBER 6, 2023. The licensing coordinator’s review of four staff files revealed that one staff had 14 ½ hours of professional development in 2023 and one staff had 16 ½ hours of professional development in 2023, when 18 hours of professional development are required annually. This constitutes the second repeat citation and failure to comply with two approved corrective action plans regarding staff not maintaining professional development hours. Corrective action: Both staff members have now completed their 18 hours of PD. Our plan in our last citation was to close the center down one day in August and have our staff complete their PD hours in one day (the last citation was in 10/2023 and we do have the PD day planned with workshops, etc which is scheduled for 8/26/24 in which all staff will still complete hours). One of the staff members that was short some PD hours was enrolled in 2 ECE courses and I had told her the 2 classes would cover her PD hours for the year, however she did not pass these 2 classes as of the end of May and was working on her PD hours (she has completed them). In the future, staff will not be able to count their ECE courses as PD hours until the course is completed and staff are able to show they have passed the course. The director has also downloaded an app to make tracking staff members PD hours and expiring PD hours for the future.
This Statement of Findings includes a repeat citation for He-C 4002.33(f), which constitutes the second repeat citation and failure to comply with two approved corrective action plans, as noted herein. Corrective action: The director has downloaded an app on her phone do we will be alerted each month when a staff members PD hours expire so we are able to address the PD hours more accurately when they expire. In the future, we will be in compliance with the 18 hours of PD hours as the app will allow more accurate dates of PD hours expiring. Moving forward the program will be more aware of following approved CAPS.
Staff members stated that the program uses a bell to signal fire drills, when the actual fire alarm system for the building is required. Corrective action: Moving forward, we will sound the fire alarm system for each fire drill.
Two licensing coordinators' review of five staff files revealed that there was no documentation of training and professional development for one staff. The staff member stated that that they did complete professional development in 2022, however did not retain the documentation in their file. Corrective action: The staff that did not have any professional development hours on file had thrown the printed hours away thinking we are in 2023, we only need the 2023 hours, not realizing BCCL would be going back a year, moving forward, we will be keeping all old and expired pd hours in staff files.
The licensing coordinator's review of department records revealed that one staff who was working in the program on the day of the visit was not deemed eligible. The center director confirmed that the staff has been working in the program. The program was previously cited for this violation following the February 26, 2021 visit. A previous corrective action plan submitted by the program and approved by the department on May 10, 2022, stated, in part, "In the future, fingerprint based background checks will be kept current." Corrective action: The staff member was hired in July 2020 when she was not able to be fingerprinted due to the pandemic. When our licensor visited in April 2022 we were told to have her fingerprinted as the order had been lifted. The staff member went for fingerprinting but I did not realize that she had to re-submit her H&P, state background check and EEC so the fingerprinting was kicked out of the system. The staff member was sent again for fingerprinting on 10/3/23 and we re-submitted all of the paperwork. Unfortunately when the licensor visited on 10/5/23 the BRC results were not in yet. The staff member was approved of her BRC status on 10/9/23. The center director will check the background checks and eligibility of each staff member including the 5 year re-evals on a monthly basis when we have /plan our staff meetings to ensure that brc's are up to date.
Two licensing coordinators review' of five staff files revealed that one staff had zero hours of professional development in 2022, when 18 hours of professional development are required annually. A previous corrective action plan submitted by the program and approved by the department on May 10, 2022, stated, in part, "In the future, the center director will ensure that staff are completing their required hours of professional development hours." Corrective action: Starting in 2024, we will be closing our center for a full day of professional development/trainings for our staff to ensure they are able to get their 18 yearly hours. The entire day will be dedicated to professional development trainings. Staff and the center director will track the staff development hours on a spreadsheet in the staff's files and we will review everyone's hours at our monthly staff meetings.
