The last annual compliance visit was conducted on August 26, 2025. The facility operates with a Five Star License, which was issued on July 5, 2017. Upon arrival I was greeted by Tyshia McCain, Center Director The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent (100%) as of 9-9-25. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in outdoor time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Children’s files were reviewed today (ten percent of enrollment). Fire (8/26/25) and sanitation (6/18/25) inspections remain current. Playground inspection:8/1/25. Emergency drill log: Shelter and lock down drills will be conducted in October 2025. Fire drill: 8/29/25. A checklist was used to note the requirements I monitored today. One violation was cited during today’s visit; This violation was corrected during the visit. The provider did ask questions to ensure the facility stays in compliance. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The operator did schedule a fire inspection within 12 months of the previous inspection, however the fire inspection still wasn't completed until 8/26/25. The inspection also was not on the form provided by the division, due to the inspector not using the form provided by the division. The previous inspection was conducted on 6//3/24. 10A NCAC 09 .0304(a) Technical Assistance I also discussed QRIS and what options you will have regarding the three Pathways. Please go to our website DCDEE to become knowledgeable about this change that came into effect July 1, 2025. Please go to the NCRLAP website, www.ncrlap.org, to learn more about the third edition of the scale booklets and the Environmental Rating Scale Process. The third edition of the scales will be used for all assessments conducted. Make sure all new staff have updated WORKS accounts to reflect on all current and completed coursework. (DCDEE-WORKS) RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These training courses are free, and participants will receive training certificates when completed. Upon arrival to the facility I was greeted by Tyshia McCain, Center Director. We conducted a walkthrough of the facility. Attendance was taken and observations made. Children were observed participating in indoor play, outdoor free play, teacher director activities and routines. A fire drill was conducted during today's visit. The last Fire Inspection was conducted on May 24, 2023 and approved for day time care only. The center's last Sanitation Inspection was conducted on May 18, 2023 and a 'Superior' classification was received During today's visit, a full assessment of applicable Child Care Requirements was conducted. Permit posted, supervision, staff/child ratio, program records, children’s records and staff files. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 1314 Emergency information did not name Child's health care professional. One child's file did not contain the name of the medical professional and one child's file did not contain the choice of the hospital preference. .0802(c)(2) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A medical file was present for each staff member, however the annual health questionnaire was maintained in the personnel file. .0701(d) The above violations must be corrected immediately, and a letter sent to the office stating how the violations were corrected. Please include with the letter all documentation to support the corrective actions taken. Your letter should be received by the DCDEE office on or before September 11, 2023. You may email the letter to my supervisor Kaye Adkins at, kaye.adkins@dhhs.nc.gov. Please “cc” a copy to me at anita.c.grahm@dhhs.nc.gov.
