This visit was conducted with Jeanette Maines, Director. Your program currently operates with a GS 110 Notice of Compliance effective 5/1/2000. Restrictions include daytime care only; No more than 10 less than three (3) years old. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children listening to a Halloween story at group time, having Lego manipulative play with teachers, and infants having induvial needs met. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 11/25/24. A sanitation inspection was completed on 6/13/25 with a Superior classification and eight (8) demerits. The last fire inspection was conducted 8/12/25 by Daniel Roten. Program records and required postings were monitored. A fire drill was conducted on 9/18/25 at 10 am with three (3) minutes to evacuate fourteen (14) children and two (2) adults by J. Maines. A shelter in place emergency drill was documented on 9/17/25 at 9:30 am with three (3) minutes to shelter-in-place by J. Maines. An outdoor inspection was documented on 10/10/25 with no issues noted on the playground inspections from. One (1) new staff file and one (1) regular staff file and two (2) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. There is one child with Tylenol and required medication paperwork was monitored. The program does not provide transportation. The following violation was cited during today’s visit: Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One new staff member had a TB test with the date of 5/12/2022, which is older than 12 months. .0701(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Jeanette Maines assisted me with the visit. Your program currently operates with a Notice of Compliance effective 5/1/2000. Restrictions include 1st shift day time care only; No more than 10 less than three (3) years old. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license (Notice of Compliance) was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice time on the outdoor playground and transitioning back into the center. No new staff were reported at this program, so no files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. No diaper creams or ointments were currently at the center. Storage hazardous items was monitored today. A playground inspection was recorded 7/9/24 with no items noted in the playground inspection sheet. A fire drill was recorded 7/17/24 at 8:50 am with two (2) minutes thirty (30) seconds to evacuate twenty (20) children and three (3) adults. A shelter in place drill for the facility was last recorded 7/19/24 at 10:30 am with three (3) minutes to shelter-in-place by J. Maines. The most recent fire inspection was on 5/11/23 by Chris Nichols. I observed a sanitation inspection was last conducted on 4/15/24 earning a Superior classification with four (4) demerits. You stated that you do not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. The safe sleep policy was posted however the boxes were not checked as to the procedures this center follows. I observed no documentation of sleep checks for the date of Friday, August 2, 2024. 10A NCAC 09 .0606(a) 891 The safe sleep policy did not contain the required information. Jeanette Maines assisted me with the visit. Your program currently operates with a Notice of Compliance effective 5/1/2000. Restrictions include 1st shift day time care only; No more than 10 less than three (3) years old. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license (Notice of Compliance) was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice time on the outdoor playground and transitioning back into the center. No new staff were reported at this program, so no files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. No diaper creams or ointments were currently at the center. Storage hazardous items was monitored today. A playground inspection was recorded 7/9/24 with no items noted in the playground inspection sheet. A fire drill was recorded 7/17/24 at 8:50 am with two (2) minutes thirty (30) seconds to evacuate twenty (20) children and three (3) adults. A shelter in place drill for the facility was last recorded 7/19/24 at 10:30 am with three (3) minutes to shelter-in-place by J. Maines. The most recent fire inspection was on 5/11/23 by Chris Nichols. I observed a sanitation inspection was last conducted on 4/15/24 earning a Superior classification with four (4) demerits. You stated that you do not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. The safe sleep policy was posted however the boxes were not checked as to the procedures this center follows. I observed no documentation of sleep checks for the date of Friday, August 2, 2024. 10A NCAC 09 .0606(a) 891 The safe sleep policy did not contain the required information. The boxes were not checked on the safe sleep policy as to what this center chooses to do for safe sleep checks. 10A NCAC 09 .0606(a)(1-8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, August 20, 2024. Restrictions include day time care only; No more than 10 less than three (3) years old. The Notice of Compliance was observed, and the restrictions were found in compliance. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. The last annual compliance visit was conducted on 1/23/23. A sanitation inspection was completed with a 10/25/23 with a Superior classification and two (2) demerits. The last fire inspection was conducted 1/27/23. Program records and required postings were monitored. A fire drill was conducted on 11/15/23 at 10:30 am with three (3) minutes to evacuate sixteen (16) children and three (3) adults. A lockdown drill was documented on 11/29/23 with three (3) minutes to lockdown. An outdoor inspection was documented on 11/29/23. Removing leaves was the only item noted on the playground inspection checklist. The leaves had been removed. Church staff help with maintenance. Staff files and children’s records were monitored per DCDEE procedures. One staff file was reviewed during today’s visit. No violations were cited in the staff file. Three (3) children’s files were monitored today. No violations were cited in the children’s files. Nutrition requirements were monitored per DCDEE procedures.
