This visit was conducted with Debbie Crouse, Director. Your program currently operates with a FIVE Star license issued 10/30/17. Restrictions include 1st shift daytime care; meets enhanced ratios minus one; meets enhanced space. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center play, circle/group time, and outdoor gross motor play. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on 1/8/25. A sanitation inspection was completed on 6/23/25 with a Superior classification and thirteen (13) demerits. The last fire inspection was conducted 9/23/25. Program records and required postings were monitored. A fire drill was conducted on 10/24/25 at 10:43 am with two (2) minutes, fourteen (14) seconds to evacuate twenty-one (21) children and seven (7) adults by D. Crouse. A lockdown emergency drill was documented on 10/23/25 at 7:26 am with forty-seven (47) seconds to lockdown by D. Crouse. An outdoor inspection was conducted on 10/24/25. Hazardous items were monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The ground level at the base of the cushioned playground surface is lower due to washing away of the dirt during Hurricane Helene. This poses a tripping hazard to the children when playing on the playground outside the glass windows. 10A NCAC 09 .0601(a) 866 A caregiver administered medication to a child without parental permission or authorization from a bona fide medical care provider during the event of an emergency medical condition when the child's parent was not available. Three (3) medications were found in Space 2 with no permission to administer the medications. 10A NCAC 09 .0803(10) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A fire drill was recorded 6/21/24 at 10:11 am with two (2) minutes, forty-four (44) seconds to evacuate twenty-one (21) children and six (6) adults by D. Crouse. A lockdown drill for the facility was last recorded 5/10/24 at 8:04 am with one (1) minute, two (2) seconds to lockdown by D. Crouse. The most recent fire inspection was on 2/23/24. I observed a sanitation inspection was last conducted on 4/25/24 earning a Superior classification with thirteen (13) demerits. You stated that you do not provide transportation. The following violation was cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. I observed plant growth coming through the fence line on the large playground. This is a hazadrd to the children as some plants may be poisonous to children. Visibility is limited to seeing any vermin coming through the fence that would be hazardous to childrenon the playground. A sanitation inspection was completed 12/12/23 with a Superior classification and thirteen (13) demerits. The last fire inspection was conducted on 1/27/23 and a copy was received today during the visit. Program records and required postings were monitored. A fire drill was conducted on 1/12/24 at 9:34 am with one (1) minute and fifty-nine (59) seconds to evacuate eighteen (18) children and seven (7) adults by D. Crouse. A lockdown drill was documented on 11/29/23 at 2:58 pm with one (1) minute, nineteen (19) seconds to lockdown by D. Crouse. The NC Secretary of State website was reviewed on 1/26/24 and Blue Ridge Developmental Day, Inc. was listed as current and active. The program’s compliance history was 88% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time and having lunch and nap time. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. A sanitation inspection was completed 12/12/23 with a Superior classification and thirteen (13) demerits. The last fire inspection was conducted on 1/27/23 and a copy was received today during the visit. Program records and required postings were monitored. A fire drill was conducted on 1/12/24 at 9:34 am with one (1) minute and fifty-nine (59) seconds to evacuate eighteen (18) children and seven (7) adults by D. Crouse. A lockdown drill was documented on 11/29/23 at 2:58 pm with one (1) minute, nineteen (19) seconds to lockdown by D. Crouse.
