The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. You, J. Sheilds, Operator was present and available to assist with today’s visit. Your program currently operates with a four-star license issued January 21, 2020, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two (2) children under one year of age. The last annual compliance visit was conducted on September 18, 2025. The center’s compliance history was reviewed with the operator. The program’s compliance history was 100% percent as of March 9, 2026. Two children were present during the visit. The children were observed playing with developmentally appropriate toys and the operator was interacting in caring manner and meeting the developmental needs of the children. HOUSEHOLD MEMBER(S) You stated you are the only person who lives in the home. TRASNPORTATION: You stated you do not provide transportation. The following were monitored: Capacity, First Aid, CPR, ITS SIDS, CBC Qualification, General Health, Storage of Hazardous Substances, Administration of Medication, General Safety, General License Requirements, License Posted, Fire Safety, Program Records. No violations were documented during today’s visit. Violation Number Comment Rule 305 Home was not equipped with either an electrically operated smoke detector with a battery back-up; or both an electrically operated detector and a battery operated detector located next to each other. The home had an electrical smoke detector; however, it was not operating correctly. NCAC 513.1; .1707(3) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 24, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. This time frame is not intended to be used as a guide for correcting violations, as they should be corrected immediately. OWNER/FACILITY PROFILE INFORMATION: You, J. Sheilds, are listed as the Sole Proprietor on the Family Child Care Home’s License and you stated during today’s visit this ownership remains the same. Prior to changing the facility name and/or ownership (i.e., forming a corporation), please contact me to discuss procedures. Failure to comply may affect your Family Child Care Home License. You verified the mailing address, phone number, and email address listed on the facility profile are correct. TECHNICAL ASSISTANCE/REMINDERS: ELECTRICAL SMOKE DETECTOR: The operator had an electrical smoke detector and a battery operated smoke detector; however, the electrical smoke detector was not operating correctly. When Ms. Shields tried to test to it was not turning on, then it did turn on, but was not making the noise it should. It was also making an noise when not in testing mode. It sounds like their may be some type of electrical short in the system. Ms. Shields stated should would have an electrician come out to take a look at it. QRIS UPDATE: On October 22, 2025, we discussed transitioning your license to using Pathway Two: Classroom and Instructional Quality to maintain your four-star license. You are currently working on gathering documentation for the family engagement and continuous quality improvement plan. You are currently using Creative Curriculum and are looking for a formative assessment tool to use. We reviewed approved curriculums, approved formative assessments, your education, and on-going training. You are required to receive five (5) hours of on-going training annually and you will either need to complete another five (5) hours of on-going training or .5 CEU’s. ABCMS TRAINING: You have completed the ABCMS Moodle training, and your portal is current. REMINDERS: CLEAN WATER FOR CAROLINA KIDS: According to the Clean Water for US Kids website, you tested your water on November 14, 2024. RTI is reviewing your lead paint survey, and your asbestos survey.
