Your facility operates as an NC PreK program in the following spaces: five, where children participating in the NC Pre-K program are cared for. Kim Barnes, Child Care Consultant, assisted with today's visit. Upon arrival, You, C. Arrington, Administrator, was present and available to assist with today’s visit. Your program currently operates with a five-star license issued on June 27, 2025, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for the Administrator has completed a business training course of at least 30 clock hours. The last annual compliance visit was conducted on June 3, 2025. The last sanitation inspection was completed on December 31, 2025, with a “Superior” classification “0” demerits. The last fire inspection was conducted on April 30, 2026, and your facility was approved for daytime and night care. The license was posted, and the restrictions were in compliance. Thirty-one (31) children were present from infants to five years old. The children were observed in free-play in centers, music and movement, engaging in gross-motor play, in the outdoors, and personal care routines. Staff were observed interacting with the children in a positive and nurturing manner. The children were served the following for lunch: pizza, green beans, mixed fruit and milk. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations were documented during this visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed on April 30, 2026; however, the completed inspection was not sent to the child care consultant within seven (7) days of the inspection being completed. 10A NCAC 09 .0304(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The Ford Passenger Van with license plate DKB-2713, had a fire extinguisher on the vehicle; however, the fire extinguisher was not operable, it was in the recharge zone. 10A NCAC 09 .1003(c) 1915 Aquatic activities were permitted in hot tubs, spas, saunas or steam rooms, portable wading pools, natural bodies of water, or other unfiltered non disinfected containments of water. A A small plastic portable wading pool was being used by preschool age children to dip cups and splash each other for water play. .1403(b)(1-5) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 3, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. This time frame is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware that any written information submitted by you regarding correction of violations documented during today's visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Please be aware that if it is determined that information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's license could be issued. COMPLIANCE HISTORY: Prior to conducting today’s visit your center’s Compliance History score, over an 18-month time frame, is 96%. The violations documented could impact the center’s overall Compliance History score. Your compliance score must be maintained at 75% or higher or an Administrative Action may be warranted. TECHNICAL ASSISTANCE/CONSULTATION: ANNUAL FIRE INSPECTION: The fire inspection was completed on April 30, 2026; however, the completed inspection was not sent to the child care consultant within seven (7) days of the inspection being completed. Please remember fire inspections must be sent to the child care consultant with seven (7) days of completion. This was corrected during the visit. This was corrected during the visit. AQUATIC ACTIVITIES: Small plastic portable wading pool was being used by preschool age children to dip cups and splash each other for water play. The children were not wading inside the pool. Informed Director wading pools are prohibited on child care premises. Wading pool was emptied and removed from the playground. This was corrected during the visit. TRANSPORTATION: The Ford Passenger Van with license plate DKB-2713, had a fire extinguisher on the vehicle; however, the fire extinguisher was not operable, it was in the recharge zone. You contacted fire extinguisher service company to have the fire extinguisher serviced. STAR RATED LICENSE: You have decided to complete Pathway One: Program Assessment. Originally you wanted to complete Pathway Two but didn’t realized you would have to reduce your ratios by at least one child to meet the required reduced enhanced ratios. We reviewed the NC Rated License Assessment Project (NCRLAP) and reviewed the self-study forms. You are going to be working on your self-study from May through August and request the environment rating scales in August. REMINDERS: CLEAN WATER FOR US KIDS: We discussed the Clean Water for Carolina Kids program. According to the Clean Water for Us Kids website you last tested the drinking water on March 27, 2024. Please remember child care facilities must test their drinking and cooking water every three (3) years. Lead was tested March 29, 2024, and Asbestos June 27, 2024. ABCMS ROSTER: You have completed the ABCMS roster. Please continue to keep it updated by deleting staff that no longer work for you and add new staff within five days of employment.
