Elizabeth Childress, Child Care Consultant, accompanied me on this visit. A checklist was used to note requirements we monitored today. This facility has a 100% compliance score as of 10/6/25. The last annual compliance visit was on 10/17/24. This child care center currently operates with a Five-Star License, effective 4/24/18. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. A sanitation inspection was conducted on 5/7/25, with a superior classification and 2 demerits. A fire inspection was conducted on 3/27/25. Upon my arrival, we were greeted by, D Walton, Administrator for the program. Ms. Walton assisted us with the information required during the visit. We completed a walkthrough of the licensed spaces and noted attendance. Emergency drills and outdoor inspections were current and completed by trained staff. Required items were posted and accessible. A full assessment of applicable child care requirements was conducted during today’s visit. We completed a walk-through of the indoor and outdoor areas that the children use. Forty-nine children were present. I observed the children napping, playing in centers, playing outside and participating in group time. Teacher interactions were positive and nurturing. Medications were checked in each room and paperwork was correctly completed for each medication. We reviewed 5 children files. There were 2 violations of Child Care requirements observed on this visit Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member did not have results of a TB test prior to employment. .0701(a) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected . It must be submitted to me by October 22, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected. We provided technical assistance regarding: Staff need a medical report and TB test on file prior to employment. It is important to have all staff qualifications documented in NC Works as this is what will be used to determine education standards on the Star Rated License. The Workforce Education Unit has recently updated and added resource documents to assist individuals with all Workforce Education Unit processes. During today's visit, staff in the infant classroom had their It's-SIDS training. We reviewed the rule regarding It's SIDS. As a reminder, any staff who work in the infant room need Its-SIDS within 60 days, however, someone with It's-SIDS always needs to be present in the infant room. Everyone in the classroom needs to be aware of the Safe Sleep Policy and Sleep charts. We discussed QRIS Pathways. You are currently leaning towards Pathway #1.
