On 8/15/2025, Child A was under the supervision of child care staff.
On 8/26/2025, Child L’s diaper was changed twice, and he urinated in the potty twice. His diaper was changed when teachers noticed he was wet or soiled.
Since 2/5/2025 a pest control service has been inspecting the building, setting traps and glue board, and making recommendations every two months to eliminate/prevent rodents. Repairs were made to the building on 6/26/2025 and 9/13/2025 to potentially prevent rodents from entering the building. The premises are being maintained to eliminate and prevent rodent activity.

On March 27, 2025, Child Care Staff Member 1 aggressively grabbed Child B's neckline of his shirt and pulled him towards her while appearing to then get into Child B's face.
Program Director did not inform Child B's Mother within 24 hours of the knowledge that Child B had been inappropriately grabbed by CCSM 1. Program Director waited more than 7 days to inform Child B's Mother.
On March 21, 2025, during the afternoon programming CCSM 1 and CCSM 2 were not appropriately supervising or interacting with the children in care rather they were sitting using electronic devices.

On March 10, 2023, I received a self-report incident report from Program Director Jenny Buechel at Childtime in Shelby Twp. Ms. Buechel indicated that late in the day on March 8, 2023, Child A's Mother was picking Child A up from care, saw him crying, and asked Child Care Staff Member 1 (CCSM) what had happened to make him cry. There was an explanation given by CCSM 1 to Child A's Mother and Child A left with his mother. A brief time later CCSM 1 came into the office and told Ms. Buechel that Child A's Mother blamed her for hitting Child A. CCSM 1 told Ms. Buechel that she did not hit 2 Child A either purposely or accidently. Child A was upset because he had been told that he could not ride a bike because of his behavior. When Child A's Mother arrived, she saw Child A reacting to the news. He was whimpering when his mother arrived. When Child A's Mother became confrontative with CCSM 1, he started crying. The next day, March 9, 2023, Child A's Father arrived back at the facility to complain that Child A had told him that CCSM 1 had hit him. Child A's Father had a picture of one side of Child A's face showing a red cheek. Ms. Buechel said that since the picture was only of one side of Child A's face, she could not compare the cheeks together. There was no indication of when the picture was taken. Considering the outside temperature that day and that Child A had been crying there were other explanations for red cheeks. Child A's Father terminated Child A's enrollment that day. A copy of the picture of Child A's cheek was not given to Ms. Buechel or Ms. Cole. Ms. Buechel was interviewed on March 10, 2023, by Officer Nye from the Shelby Township Police Department. (# 23-9016) He explained that a complaint was made about CCSM 1 hitting Child A and he would be forwarding the information on to the Detective's Bureau. I requested that Ms. Buechel contact Area Manager April Cole to save any video surveillance of the time when the alleged incident took place. Ms. Cole responded, and we set up an appointment for me to watch the video at the center. On March 22, 2023, I arrived at the center for a scheduled inspection. Upon arrival, I met with Ms. Buechel. We went over her original explanation of events — Child A's Mother arrived around 3:25pm to pick Child A up from his Great Start to Quality class (GSRP). That is when that program's scheduled day ends. Parents/Caregivers pick their children from the outdoor play area. Child A left with his mother shortly after she arrived to pick him up. Ms. Buechel stated that Child A's Mother never stopped to talk to her (Ms. Buechel) before leaving. Ms. Buechel saw her leave the building. CCSM 1 came into the office where Ms. Buechel was around 3:40pm. CCSM 1 told her the story about what had occurred on the playground when Child A's Mother came to pick Child A up. The rest of the day continued without hearing from Child A's parents. The next day Child A's Father came into the building complaining that CCSM 1 had hit Child A on the face. As his evidence he showed a picture of only one cheek on Child A's face; not Child A's whole face to compare the cheeks. As stated earlier in the investigation, there were other factors why Child A's cheek could have been red. Child A's Father terminated Child A's enrollment at the center and proceeded to make a police complainant when he felt the center administration was not looking into the incident. Ms. Buechel stated that she had informed Ms. Cole about the 3 incident on March 9, 2023, and the surveillance footage from two outdoor cameras was pulled for review. It was implied by Child A's parents that Child A had been hit on the face while he had been on the playground and before his mother arrived. That was the footage that was pulled for my review on March 22, 2023. Ms. Cole arrived at the center and set up the camera footage on the computer screen for my review. Ms. Cole explained that Officer Nye had already reviewed that specific footage on March 10, 2023.
On April 7, 2022, Ms. Daughtry was alone with 12 children ages 3 years and older. She was out of ratio for approximately 30 minutes.
During the investigation Ms. Ringwelski admitted that she took Child B to the center's office several times for care for periods of 10 minutes. The office is not an approved child care space area.
On April 8, 2022, Child Care Licensing received allegations of inappropriate supervision and care at Childtime child care center. The written complaint alleged that Child A had been physically injured by Child B on multiple occasions with caregiving staff failing to intercede or address Child B's behavioral problems. The complainant wrote that he observed the available live video feed of the room and could see that Child B was repeatedly disruptive in class and was very physical with other children (names unknown) during child care hours. The 3 caregivers present watching Child B minimized his behavior when questioned by the complainant. After receiving the complaint, I received notification from Department of Health and Human Services Worker (DHHS) Christina Adamic that her department would also be investigating an alleged incident that occurred on April 7, 2022. On April 8, 2022, I contacted program director Beth Kueppers at Childtime. She stated that she was aware of the allegations and gave a brief description outlining the incident on April 7, 2022, and previous documented incidents involving Child B. On April 7, 2022, Ms. Kueppers was not at the center when Child A's Father came to pick him up from care. It was the Assistant program director Kallie Ringwelski who was at the center. According to what Ms. Kueppers was told, Child A's Father was upset when he picked Child A up due to scratches on the back of Child A's neck. Child A's Father wanted to know what had happened. This was at closing and Ms. Ringwelski was unable to give him an answer or find an incident report. She closed the building after Child A's Father left. When Ms. Ringwelski was leaving work, she passed a police vehicle pulling into the parking lot. The next day Ms. Kueppers was advised of the situation and notified the covering Area Director, Jen Gunderson, about the incident and requested the video of the day be saved. I also requested Ms. Kueppers to forward me an email address to Ms. Gunderson so I could make the same request. Ms. Kueppers explained that there have been past incidents of alleged poor behavior on the part of Child B to Child A. Child A's family has described the behavior with "the little black boy." The staff was aware of the behavior and was working with the behavioral team with the Learning Corp. Ms. Adamic and I scheduled a joint interview with Ms. Kueppers for April 13, 2022. Upon arrival we met with Ms. Kueppers and immediately requested all written records regarding Child A and Child B. Ms. Kueppers provided their respective files to us as we sat down for the interview. While Ms. Adamic and I reviewed the paperwork Ms. Kueppers went into greater detail about Child B's behavior. She described Child B climbing and getting over a fence in the outdoor play area right in front of staff, physicalness of his behavior with other children and staff, and biting behaviors. A review of the records documented 34 incident reports for Child B alone with 14 reports involving him biting other children and staff. This was since January 21, 2022, until the day of the interview. The incident report dated 10/8/21 details how Child B was found in a bathroom stall looking at Child C's (3-year-old female) body while she had her pants down. These stalls are private stalls made for individual children. Child B should not have been in the same stall. 4 Ms. Kueppers stated that due to the amount of behavioral incident reports she connected with the company's behavioral team to address issues and guidance. The contact person helping her was DaChardea Roncoli. As of the interview date Ms. Roncoli has supplied written pamphlets with advice about behavior to staff. When asked by both me and Ms. Adamic what the end goal was regarding Child B's behavior Ms. Kueppers did not provide a clear answer. As for the alleged incident on April 7, 2002, Ms. Kueppers provided the following information that she learned during her brief interviews with Child Care Staff Members
On August 3, 2021, Assistant Program Director Kalie Brown from Childtime self-reported an incident of inappropriate supervision of Child A (3 1/2 year-old female) by telephone. It was followed up by a written incident report on August 5, 2021. 2 On August 9, 2021, I spoke with Program Director, Beth Kueppers, who provided more detail about the incident. She explained that the preschool room was getting ready to go outside under the supervision of Child Care Staff Member (CCSM) Elizabeth Poppe. At approximately 12:15pm on August 2, 2021, Ms. Poppe had the children line up, conducted a head count, and walked out of the room leaving Child A behind and alone. The Lead Teacher CCSM Sarah McCormick walked into the preschool room and found Child A by herself and visibly upset. Ms. McCormick brought her to Ms. Brown in the office. Ms. Kueppers was not present in the building at the time the incident occurred. Since the incident, Ms. Kueppers has begun to re-train CCSMs in name to face head counts and the policies about steps taken before leaving the building. On August 11, 2021, I conducted an unannounced inspection at Childtime. Upon arrival I met with Ms. Kueppers. She stated that since the incident CCSM Ms. Poppe, who is a new employee, was put on a final write-up. The video surveillance of the incident was no longer available, but District Manager April Cole reviewed it to make the decision of a write-up. It was Ms. Kuepper's understanding from both Ms. Brown and Child A's Mother that Child A's Father was informed that same day of the incident August 2, 2021. Ms. Kueppers stated she was off from work during week of the incident. She had to explain to Ms. Brown by telephone to call Licensing to report the incident verbally then write up an incident form. (The written incident report had to be revised by Ms. Brown a couple times to provide more information before I would accept the form as final.) As reported by Ms. Kueppers the incident on August 8, 2021, went as follows: Ms. Poppe was in the preschool room that day well within ratio of 1 caregiver/10 children. At 9:45AM Child A arrived in the room. The rest of the morning went as scheduled. (Ms. McCormick was not in the room that day due to low staffing so, she was needed in another capacity.) Ms. Poppe was in the room with 10 children getting ready to go outside at around 12PM. Ms. Poppe had the children line up, conducted a count, and left the room to go outside. Ms. McCormick went to the room for an item and found Child A by herself and visibly upset but not physically hurt. Ms. McCormick took Child A to Ms. Brown. Ms. Brown calmed Child A down in the front office for several minutes. Ms. McCormick went outside to let Ms. Poppe know that Child A had been left inside alone. Ms. Poppe had not realized that Child A had been left in the room. I interviewed Assistant Director Kalie Brown in the office. She explained that she was covering the program director role while Ms. Kueppers was off. At about 12:15PM on August 2, 2021, Ms. McCormick came in the office with Child A who was visibly upset. Ms. McCormick told Ms. Brown that she found Child A alone in the preschool room. Ms. McCormick left Child A with Ms. Brown to find Ms. Poppe. Ms. 3 Brown calmed Child A down enough to have her rejoin her class. She then notified both Ms. Cole and Ms. Kueppers about the incident. They gave her the instructions to notify both Licensing and Child A's Mother. Prior to leaving the building Ms. Kueppers stated that both Ms. Poppe and Ms. McCormick were not working in the building that day. She would let me know when both CCSMs were scheduled to work. On August 12, 2021, I received an emailed written statement from District Manager April Cole. Ms. Cole stated that she reviewed the surveillance footage and saw "Child A was in line at the door to go outside to the playground." "Ms. Poppe did the initial face to name ratio check then went out the classroom door first and the children followed." "Child A
CONCLUSION: REQUIREMENT NOT MET CORRECTIVE ACTION PLAN: esponse: To ensure that all staff files contain signed abuse and neglect acknowledgment forms as required for licensing, I will implement the following procedures: Onboarding Documentation: All staff will be required to review and sign the abuse and neglect acknowledgment form before beginning employment. Organized Filing System: Staff files will be organized with clearly labeled 3 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP sections, including one specifically for signed policy acknowledgments such as the abuse and neglect form. This ensures quick access and verification during licensing inspections. File Audits: Regular internal audits of staff files will be conducted to confirm that all necessary documents, including abuse and neglect acknowledgment forms, are complete and up to date. During the inspection I observed one infant caregiver in the lobby area with four children in a stroller. Upon entering the room, I observed five infants, one caregiver, and two school-aged children who were not documented on the room attendance sheet. In the toddler room I observed 13 children (the oldest in the infant room had been brought to the toddler room) and one caregiver. A second caregiver had left the room to find the third caregiver because a child had woken up from nap. That still left the room over ratio by one child.
