R 400.8125(1) Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.
On June 15, 2021 and l made face-to-face contact with Child B a at residence and spoke with mother and father. Child B's Mother stated Child B ha started attending the child care center on . On June 8 and June 9, 2021 attended for shorter days and then on June 10 and June 11, 2021, atten a full days. On June 8, 2021, Child B's Mother noticed scratches on ild B's 3 0 legs. She stated she clipped, nails but came home with more scratches the next day. She received incident reports from the center concerning the scratches. On June 10, 2021, Child B's Mother reported she noticed "some purplish spots" on Child B on chest near the left shoulder and on back (left side) nears c.pine. On the evening o June 11, 2021, she noticed the a spots were yellowing and she realized the spots pur were bruises. She took photographs of the s ots and bruises, had received copies of the photographs. I asked to send me the photographs. Child B's Mother stated that on June 14, 2021, she went to the child care center to speak with the program director, Cassandra Chartier. The program director was not present at the center and she was told by a staff member that Ms. Chartier would call her. Child B*s Mother stated she received a phone call from Ms. Chartier shortly after leaving the center. Child B's Mother said she informed Ms. Chartier of the bruises she observed on Child B and wondered if anything had happened to Child B while was in care that would cause the bruises, Ms. Chartier told her she was not aware of any injuries or incidents and asked her for the details of where the bruises were and timeline of when they were observed. Child B's Father stated that Child B likes to stand or lunge and wonders if a staff member may have grabbed ■ hard to keep from falling while holding . Child B's Mother took Child B to see the doctor on une 15, 2021. The doctor to er the bruises nt report. The incident reports Child Bas Mother provided document that on June 8, 2021, staff noticed scratches on Child B's chin and on June 10, 20211,, Child B scratched pretty good" on the chest. Both incidents state they occurred when Child B woe up from a nap. I requested copies of the incident reports from the center. I received these on June 23, 2021' . The copies from the center are identical to the reports provided by Child B's Mother. On June 17, 202, and I made an unannounced on-site inspection at the child careRener. We "sppoewith the acting program director, Cassandra Chartier on the telephone. She was at another location providing coverage. Ms. Chartier stated she spoke with Child B's Mother concerning the bruises she found on Child B. She has spoken with staff that would have been in the room from June 811 , 2021, and none report witnessing anything that would have caused bruising to Child B's chest or back. They all reported that Child B was "fussy' and wanted to be "held a lots due to being in care for the first time. took bottles well and liked being rocked Buta to sleep.. She informed us that staff are trained to pick up infants by placing their open hands under the "head and bottom' and then scooping the infant up. Child care staff members Jessica Cook and Kaitlynn Green both reported to Ms. Chartier that it was difficult to scoop infants from the cribs due to their stature and the height of the mattresses in the cribs. But they told her they use the "scoop" method to pick up infants 4 from cribs. None of the staff expressed any concerns about how other staff handle children. Ms. Chartier stated staff shared a concern about Ms. Cook having "long nails." Ms. Chartier stated she was aware of two incident reports that were sent home due to Child B scratching She has not witnessed any staff in the room mishandle children. She added that she has re-trained staff on how to hold and pick up infants. This was due to a previous investigation (2021D 0611010) concerning bruises being found on Child A on March 3, 2021.
