On 01/11/2023, I received an incident report indicating that, on 01/04/2023 at approximately 12:01 P.M., Child A (female, 3 years) was left alone on the playground after her class was brought inside. It stated that the children were on the playground while waiting for parents to pick up the half day children. After they were picked up, the rest of the class went into the building. Another center parent was putting her child in the car when she heard crying. She opened the gate to the play area and brought Child A to the door of the center and passed her to center staff. It was noted that Child A was not physically injured. On 01/12/2023, I interviewed Child A's Mother. The afternoon of the incident, she was called by a staff member from the center and informed of the incident. She was told that a parent found Child A and brought her inside. They estimated that she was outside for about 30 seconds. Child A seemed fine when she got picked up that day. 2 Child A told her mother that she was crying and that a grown-up brought her back in the building. Child A's Mother reported that the center is great and has great staff. She thought maybe the classroom staff were newer teachers. She stated, "Mistakes happen, and I think it's a great place." She had no concerns about sending Child A back to care and Child A had no concerns about returning to care. On 01/17/2023, I completed an inspection at the center. I spoke with program director Ms. Onica McFarland, who reported that she was working in one classroom with child care staff member Ms. Hannah Pavlich. In the other classroom were two child care staff members, Ms. Ava Wypa and Ms. Brooke Bradley, who were helping at this center from another center in the licensee group. Ms. McFarland and Ms. Pavlich were inside with their children while the other classroom of 13 children and 2 child care staff members were outside. Shortly after the other class came back inside, the doorbell rang and Ms. Pavlich answered it. Ms. Pavlich then came to Ms. McFarland with Child A and said that a parent had found her on the playground after the other class had come in. Ms. McFarland took Child A from Ms. Pavlich, took her downstairs, and told the two staff members that Child A had been left outside. She reported that they were downstairs preparing for lunch at this time. Ms. McFarland then called licensee designee Ms. Jami Young and Child B's Mother, who found Child A. Ms. Young had her send Ms. Wypa and Ms. Bradley home for the day and stated that they will not be working on this campus again. Ms. McFarland clarified that Ms. Wypa and Ms. Bradley took 13 children outside to play on the playground. They were coming back in the building as the morning-only children were being picked up, leaving them caring for 9 children. When they returned to the building, they only had 8 of these children with them. When Ms. McFarland asked them how this happened, they said they were not sure. They had been taking off children's outdoor gear and did not realize Child A was not there. Ms. McFarland reported that normal procedures are to count children as they line up or do a role call from Brightwheel. She has since reiterated these procedures with the staff. The center's front door opens to the parking lot. On one side of the parking lot is the playground, behind a tall fence. Ms. McFarland took me into the playground, where I saw that the gate latch is at the top of the inside and would be impossible for a child to open. The fence is tall enough that someone inside would not be visible from the outside, but for the small amount of ground clearance, which allowed Child B's Mother to see Child A's feet. I interviewed Ms. Pavlich, who reported that she was working upstairs with Ms. McFarland and they had two fill-in staff members downstairs. Some children get picked up around lunchtime, and the doorbell rang at this time. Ms. Pavlich answered it and it was Child B's Mother, who had just picked Child B up from Ms. Pavlich a
