On July 19, 2021, 1 conducted an unannounced on-site inspection. Ms. Haynes was not home upon my arrival. One of the child care staff members, Ms. Brittany Kibble, called Ms. Haynes to inform her I was there regarding a special investigation. When Ms. Haynes arrived, I informed her of the allegations. 2 She stated that MHM1 is hardly home. When he is home, he's not really around the child care children. MHM1 was not present during my on-site inspection. I spoke with the mother of Child B and C. She reported that she has been using Ms. Haynes child care home for several years off and on. Her children have never expressed any concerns regarding anyone in the home being physical with them. She also does not have any supervision concerns. When her children are not scheduled to attend child care they are asking to go. Child A's Mother and Child D's Mother have not returned my phone call.

Ms. Haynes has two child care staff members that have been determined eligible by a comprehensive background check to provide care for child care children. During my on-site inspection Ms. Kibble and Ms. Lornez were present along with Ms. Haynes. Child B and C's Mother stated that either Ms. Haynes or Ms. Kibble are there when she drops off her children as well as when she picks them up daily. Both child care staff members were present at the time of my on-site inspection. 3

I spoke with the child care staff member Ms. Brittany Kibble. She stated that there were two child care staff members present with 8 child care children. There are always at least two child care staff members present in the child care home and that she is never alone with more than 6 child care children. During the on-site inspection three additional child care children and Ms. Haynes arrived at the home. I reviewed Ms. Haynes attendance records for the week of the July 12, 2021. The documents showed that on July 14, 2021 and July 15, 2021 Ms. Haynes was out of ratio for 30 minutes. Ms. Haynes asked if it made a difference that some of the children belonged to child care staff member Ms. Lornez. I informed her that because she did not have documentation of what child care staff members were present, I was not able to establish that she was within ratio. 4

I completed an onsite inspection at the child care home on 8/26/2020. Upon my arrival. I spoke with child care staff member . She stated that she knew that we would be out because Ms. Haynes had recently contacted in regard to a child care child E making accusations about Fe stated that Child A's Mother texted Ms. Haynes on . In the text. Child A's Mother stated that Child A told Dad that MHM1 told around lunch time tha ay. Ms. Kibble-Hasenbank stated that herself, Ms. Haynes, and Mr. Haynes watched the from that day and they did not see an time that MHM1 was ever pick alone with Child A. Child A is viewed to be satin lunch; gets up from lunch and lays down for a nap until ■ Dad comes to up la er that afternoon. 2 Ms. Haynes arrived home shortly after I had arrived at the child care home. She stated that when Child A's mother texted her she immediate) asked MHM1 about the incident. sMHM1 stated that never happened and that would never do anything like that. Ms. Haynes stated that she attempted to message Child A's Mother back and tried to call her but did not receive any messages back. She then called the= to make a report because she did not want of MHM1 out in the public and she did not want to jeopardize her license. Ms. Havnes indicated that came out to their resident. She showed 5fChil Aduring lunch time so she could verify that at theme no time was MHM1 and Child A alone, recommended that Ms. Haynes not have any more contact with Child A's Mother until the was closed. Ms. Haynes, Mr. Haynes and Ms. Kibble-Hasenbank stated they were at the child care home that day and they never witnessed any between any of the children. They also commented that MHM1 has in a due to a dirt bike accident. I attempted to contact on 8/24/2020 after leaving the child care home. A message was left "wit a request for a return call.
