R 400.8170(10) Outdoor play area. An outdoor play area and any equipment located on the center's premises must be maintained in a safe condition and inspected daily before use to ensure that no hazards are present.
During the inspection I observed that the outdoor play structure with the "firemen's pole" had the pole separated from the main anchor pole with no barrier around it to stop children from playing on the structure. The cushioning material had a large hole, measuring in diameter more than 12 inches, that exposed hard packed dirt.
