Child care staff member shook Child A, (2-year-old male) aggressively and parents were not notified. On 07/21/2022, l arrived onsite As I went into the facility, we met with program director Nicole Dubose regarding the allegations. Ms. Nicole explained that she heard of the allegations on 06/27/2022, which was three days following a conversation a former employee had with the Human Resource department on 06/24/2022. During the 06/24/2022 conversation with Human Resource, the former employee expressed the desire to be compensated during a period she was off work. Ms. Nicole said she was told by Human Resources that when the former employee did not get the answer she was looking for, she switched her concerns to "Ms. Nicole having a friend at work who grabbed a child one day." The Human Resource department contacted Ms. Nicole and she investigated it immediately. The former employee told Ms. Nicole that sometime in April, she looked through the window of the toddler classroom and saw child care staff member 011ie shake Child A aggressively. Ms. Nicole said she could not prove the allegations to be true after she interviewed child care staff members as well as spoke with Child A's Mother. Ms. Nicole said Ms. 011ie always works with other child care staff members. She has never worked alone since she has been hired. I requested statements from all the child care staff members who Moorworked former employee asking if they had ever witnessed Ms. 011ie yelling, shaking or treating any of the children in an inappropriate manner. On 07/29/2022, 1 received a written statement from child care staff member, Allyson Gibbs stating that she has never see Ms. 011ie scream or hit Child A or any other child. On 07/29/2022, 1 received a written statement from child care staff member, Anne Frusher stating that she has never witnessed Ms. 011ie do anything inappropriate while working with her involving any child. On 07/129/2022, 1 received a written statement from child care staff member, Kimberly Perry stating that she has only observed Ms. 011ie instructing the students on what they should and should not do. On 0810212022 1 contacted the complainant. The complainant said sometime in April, she was looking through the window from the preschool classroom into the toddler classroom and saw child care staff member Ms. 011ie shake Child A aggressively. She believes she did it because he was not listening. I asked the complainant why she did not report the incident in April when it occurred? The complainant said she feared for her iob, so she did not reoort it right away. The complainant said she was man Kesource vetartment anci Ms. N(co[e Dubose. I asked the complainant if she saw another child care staff member in the room with Ms. 011ie when she witnessed her shaking Child A. The complainant said yes but she could not remember who it was. On 08/10212022 1 contacted 011ie Gibson. I asked Ms. 011ie if she shook Child A aggressively because he was not listening. Ms. 011ie said `absolutely not." She said she would never hurt. shake, or mishandle a child. I asked Ms. 011ie who usually CI works with her in the toddler classroom. She said she has worked in different classrooms with everyone at some point since she has been an employee. On 08/02/2022 I contacted Child A's Mother. Child A's Mother said she was aware of the allegations, and she does not believe them. She said she loves Ms. 011ie and all the staff at the center, and she has only had positive experiences with the staff. On Monday 08/08/2022, 1 contacted Child B, (7-year-old female) and Child C, (4- year-old female's) Mother regarding her interaction with Ms. 011ie as well as all the staff at the facility. Child B and C's Mother said she has never seen or heard of Ms. 011ie doing anything inappropriate to her children. On Monday 0810812022, 1 contacted Child D, (5-year-old female) and Child E, (7- year-old female's) Mother regarding her interaction with Ms. 011ie as well as all the staff at the

On 11/3/2025, Licensee Designee/Program Administrator stated during nap time, the children go in the restroom for extended periods of time without supervision. Staff did not provide appropriate supervision to Child A, Child B, and Child C when the children were left unsupervised in the bathroom for extended periods of time.
All staff and volunteers have provided appropriate care and supervision to Child A.
According to the interviews with CCSMs, parents, and onsite inspection, the center’s furniture is safe, clean and in good repair. The wooden shelf is a sturdy and able to stand alone. The wooden shelf, did not need to be mounted to a wall.
According to the interviews with CCSMs, parents, photo of the injury, and information gather during the investigation, the center provided appropriate care and supervision of Child A on 8/21/2024.
