Child A's Mother took him to the walk-in clinic and then an eye specialist. Child A was diagnosed with a conjunctival abrasion likely a result of being hit (or poked) in the eye. 3

On 02/15/2023, licensee designee Ms. Venus Cox stated Child A was picked up from care on 01/31/2023 by his mother. There was some confusion which parent picked up; however, the sign in/out sheet was brought to her office confirming Child A's Mother picked him up on 01/31/2023. On 02/02/2023 Child A arrived at school with a red eye. Staff thought it could be pink eye, so Child A's Mother was contacted. Child A's Mother picked him up and took him to the doctor. On 02/02/2023 an incident report was submitted to the bureau. On 02/15/2023, CCSM 1 stated on 01/31/2023 Child A and a friend were poking each other in the eye. CCSM 1 said she immediately approached them and asked what they were doing. Child A and his friend were giggling while playing "poke the eye." CCSM 1 explained to the children they could damage their eyes and it was not a safe game to play. CCSM 1 redirected the children to separate activities. When Child A was picked up his eyes appeared fine; otherwise, center procedures require an "ouch" report to be completed and given to parents. Child A arrived at the center for care on 02/02/2023 with a puffy and red eye. Child A's Mother said she kept him home on 02/01/2023 because his eye hurt. CCSM 1 told Child A's Mother about the game Child A and his friend were playing where the two children were poking each other's eyes. Child A's Mother said Child A told her he was hit in the eye by a friend. Child A's Mother exited the center leaving Child A in care on 02/02/2023. Staff then noticed Child A had puss in the corner of his eye, so CCSM 2 contacted Child A's Mother to let her know. Child A's Mother picked him up from care and took him to the doctor. CCSM 1 stated she submitted an incident report on 02/02/2023 once aware Child A seeked medical care. On 03/14/2023, CCSM 2 stated Child A arrived for care on 02/02/2023. CCSM 2 said staff noticed Child A had some puss in the corner of his eye, so she called Child A's Mother to inform her. Child A's Mother picked up Child A and took him to the doctor. Child A was diagnosed with an abrasion on the eye and wore an eye patch for a couple of days. CCSM 2 stated staff did not fill out an incident report on 01/31/2023 when Child A and a friend played an eye poking game because his eyes looked good, and he did not complain of any pain. Child A stayed home on 02/03/2023 and returned to care on 02/06/2023 with no eye patch. Since the incident on 01/31/2023 staff have been retrained to inform parents at pick up of any unusual events, even when it is just a game between friends. Staff have been trained to write an "ouch" report when something unusual happens. The center is also having parents initial at drop off and pick up. On 03/16/2023, Child A's Mother stated when she picked up Child A on 01/31/2023 Child A's eyes appeared fine, but just a little red. Originally Child A told her he was hit in the eye by a friend. She has since learned he was playing a game with a friend, and they were poking each other's eyes. Child A's Mother said she took him to the doctor and was diagnosed with a scratch on his eye but nothing contagious. Staff now have parents initial at drop off and pick up so there will not be any confusion. Child A's Mother has no concerns regarding the care her son receives. 4
