On 0612012019 Ms. Kathrine Wilson, child care staff member held Child A (female, years) on her cot during nap by laying her body across Child A's body and pushing her head down. On 0910212020 and I completed an onsite inspection at the center and informed licensee designee and program director Ms. Leslie Garrison of the allegations. Ms. Garrison recalled a nap time situation regarding Child A in June of 2019. However, she was not present when the incident occurred. She did not recall if Ms. Wilson told her about the incident or if she was informed by Child A's Mother the next day. Ms. eaned across Child A and rested her elbow on the cot. with no weiaht on the child. Ms. Wilson said Child A complained her neck hurt, however, Ms. Wilson reassured Ms. Garrison that she did not hurt or touch her neck. Ms. Wilson told Child A she wanted to keep her safe. Ms. Garrison stated that she followed up with Ms. Wilson and she also called Child A's Mother. She thought the issue was resolved as they agreed the plan going forward was to provide quiet activities for Child A if she did not sleep. She could not locate an incident report for the incident. Generally, at nap children will lay on their cots and child care staff members will rub their backs. If they do not fall asleep, child care staff members will encourage them to lay quietly for a short time, and then provide books or puzzles. Ms. Garrison was not aware of any "tenting" by child care staff members and has never seen any child care staff members straddling a cot. There have never been any other concerns regarding nap time procedures brought to her attention. On 09/02/2020, child care staff member Ms. Kerstyn Buchanan stated she has worked at the center for approximately years. She does not recall the incident that happened about one year ago with Chid A. She was in the room adjacent which has a connecting bathroom; however, nothing out of the ordinary happened with Child A. She was familiar with Child A and remembers that sometimes she would take naps and other times she would not. Generally, if a child is not sleeping, child care staff members will rub the child's back. If the child does not sleep, then after a few minutes staff will give the child a quiet activity such as coloring, soft blocks, super heroes, or other activities that do not make noise. When Child D (male, 5 years) was 3-years-old, his parents requested no nap for him. Staff provided quiet activities. She has never heard of "tenting" or seen any child care staff members straddling a cot. She has never witnessed child care staff members holding or pushing a child down. She has no concerns with the care child care staff members provide to children. If she saw something concerning, she would inform Ms. Garrison. On 09/09/2020, child care staff member Ms. Kathrine Wilson denied the allegations of ever pushing a child's head down. On 06/20/2019, Child A did not want to lay still or lay quietly. Child A told Ms. Wilson she was not going to lay down and screamed. Ms. Wilson walked her to her cot. Child A tried to get up several times and continued screaming. Ms. Wilson sat with her, giving her choices of laying on her stomach or back, and offered to rub her back. Child A said she wanted to get up. Ms. Wilson said she asked Child A if she was still and quiet and then told Child A no, she was not still and quiet. Child A was thrashing, rolling over, pulling the cot up and down, and bouncing. She told Child A that it was not a safe choice. Ms. Wilson demonstrated how she sat on the floor, leaned over Child A's body with her arm, with her hand on the edge of the cot. Ms. Wilson denied holding Child A's head down or hurting her neck. She told Child A she was not hurting her because she was not touching her. The next day she talked with Child A's Mother and reassured her that she did not hurt her. Once Child A realized she would not allow her to pull her cot off the floor, she calmed down. On 5/28/2021, child care staff member Ms. Kathrine Wilson failed to provide appropriate care and supervision of Child A. As a result, Child A was able to exit the Rain Forest Room unnoticed. Child A then exited the child care center and was locked out of the building for approximately 5 minutes until Witness arrived and rang the intercom button to inform the child care that they wished to enter the building. Ms. Wilson was unaware that Child A was missing until she was returned to the classroom.
