At the time of the mandatory inspection the center did not have a staff file for a substitute teacher. For correction please submit a copy of the substitutes file to the licensing specialist.
During the mandatory inspection there was a little tykes slide that was on the edge of the resilient surfacing and next to concrete. This slide needs to be moved so no children fall off the slide and onto concrete. For correction please send a photo of the slide moved.

2 children need the doctor's phone number added to their emergency form. Please forward a copy of the 2 emergency forms to the OCC when they have been filled out to their entirety.
1 child needs evidence of having had a lead blood test at 2 years of age. Please forward a copy of this child's blood lead test results to the OCC once obtained.
1 child needs an updated immunization record showing that they have received a 2nd MMR and 2nd Varicella vaccination. Please forward a photo of this child's updated immunization record to the OCC once obtained.
The director did not notify the OCC of 1 staff member (substitute) that began work in February 2025. Please forward a 1203 to the OCC so that this staff person can be added to the personnel list.
The director did not notify the OCC of 4 volunteers who left the program in June 2024. The Licensing Specialist received a staff change form (with the delete date for the 4 volunteers) during the inspection.
The Licensing Specialist observed that 1 preschool teacher only had only 10.5 hours for the February 2023-2024 time frame. and 7.5 hours of continued training for the February 2024-2025 time frame. This preschool teacher needs to obtain 6 more hours to be up to date with her continued training requirements. Please forward copies of this preschool teacher's certificates to the OCC once obtained.
A substitute has been used since a staff person left in February. The OCC did not receive a notarized Release of Information form or criminal background check results for this substitute. Therefore, this substitute was not cleared to be able to work with the children. There were 2 teachers working for whom the OCC never received a copy of a criminal background check. It was reported that the center received their copy of the criminal background check. The Licensing Specialist received the notarized Release of Information form during the inspection for the substitute. The center stated that the substitute will complete their criminal background check on June 2, 2025. During the inspection the Licensing Specialist received a 365 form to submit to CJIS to receive the OCC's copy of the criminal background checks for one of the teachers. The center stated that they would submit the 365 form for the other teacher on June 2, 2025. The director was reminded to wait for clearance verification from the OCC prior to allowing staff to work.
The center did not notify the OCC that a new substitute has been working in the child care classroom since the beginning on the school year. This individual is an employee of North Point High School; however, she will immediately need to complete the OCC's requirements such as submitting a notarized Release of Information, getting fingerprinted, and obtaining a medical evaluation. The director completed a 1203 for this substitute during the inspection. This substitute shall not be left alone with the child care children until she has been fully cleared by the OCC.
The center did not notify the OCC of the departure of one of the Early Childhood Training Program's volunteers. The director completed a 1203 for this volunteer during the inspection.
Upon inspection of the child care classroom the Licensing Specialist observed a 1 gallon jug of body lotion stored in the boy's bathroom cabinet. The jug of body lotion was thrown away during the inspection.
