Upon completing an annual Mandatory Inspection LS observed 2 staff that were not on the associated party list. LS reiterated to Director that only OCC approved child care staff should be supervising children.
Licensing Specialist conducted a Conversion Inspection today and observed 1 child without a complete Emergency Form. Facility shall maintain completed Emergency Form for all children enrolled.
While conducting Conversion Inspection Specialist observed 2 children with Part 1 of Health Assessment completed and 2 children without Part 2 of the Health Assessment completed.
Licensing Specialist observed 6 children without immunization records. Unless parents object center shall maintain immunization records for all children enrolled in facility.
