A medical evaluation for a new two year old resident has been pending since the monitoring inspection on 03/23/2026. LS also requested a medical evaluation over email. Within 15 working days after notifying the licensing agency of a new resident, the provider shall submit a medical report on the resident, on a form supplied or approved by the licensing agency that is based on a medical evaluation completed within the previous 12 months.

LS conducted a Monitoring Inspection and observed a resident present in the home that the OCC was not made aware of.
