At time of inspection, Licensing Specialist observed an attendance list that was only signed by 11/42 children enrolled and present on 08/19/2024.
At time of inspection, facility did not have an emergency form for a child present and enrolled in care. The facility contacted the child's parent to submit the form via email.
At time of inspection, Licensing Specialist observed emergency forms that were not signed and/or updated annually by the child's parent. Licensing Specialist also observed emergency forms missing the physician information.
At time of inspection, Licensing Specialist observed children's files that did not have the appropriate medical evaluation documented.
At time of inspection, Licensing Specialist observed a child's file that did not have the appropriate lead screenings documented.
At time of inspection, Licensing Specialist did not observe current/accurate staffing patterns posted in Room 4 or Room 1.
At time of inspection, Licensing Specialist observed an unknown person in the facility. OCC has not received notification or the required documents for the individual.
At time of inspection, Licensing Specialist observed the broken door closer on the door that leads to classroom upstairs. Licensing Specialist also observed peeling paint in the classrooms.
At time of inspection, Licensing Specialist did not observe paper towels in the bathroom located in the basement.
At time of inspection, Licensing Specialist did not observe a staff member supervising the children. The staff member was around the corner by the diaper changing station of the classroom away from where the children were located. The second staff member assigned to the children was in the bathroom with a child. The door that leads to the upstairs classroom was also open at the time of no supervision. Licensing Specialist spoke with a staff member and reminded them to be where the children are at all times.
