Cited finding
During a review of a sampling of children's records, it was found that five children had no physician's information whatsoever on their emergency forms. Another child did not have the physician's address or a parent signature on their emergency form. Please inform the parents of the respective children so that they may add the missing information, initial and date it to indicate the update. Please send a copy of the emergency forms with the added information to OCC for the correction of this non-compliance.
