The substitute’s original federal and state criminal background check (CBC) was completed on May 16, 2019. The 5-year reprints were due in May 2024 and have not yet been completed.

diapering mat has torn and provider is using tape to keep holes covered and mat together, mat needs to be replaced.
In bathroom was using a 6 plug piece. During inspection that piece was removed and the 2 plugs were covered
there are at least 5 pallets that are leaning against each other, per provider they were going to build something, but it did not work out. Because the pallets are leaning against each other and could fall they need to be removed from the chiid care outdoor play area.
Providers certificate in CPR & First Aid expired in May 2023.
