LS observed 4 instances of children's emergency forms that were missing various combinations of the enrollment date, schedule, physician information, and emergency contact addresses.
Licensing specialist observed a small swarm of flies located in the School Age room kitchen area as well as the connecting hallway bathrooms. Facility is asked to take out the trash, clean these areas, and take any other necessary treatment measures to make sure the flies are no longer present.

Licensing specialist observed that the list of emergency phone numbers posted at the center does not have the CPS, poison control, and local health department or physician phone numbers. Facility is asked to add these emergency phone numbers and to update the child care licensing phone number as well. The CPS reporting hotline number is 240-777-4417.
