Facility failed to notify the Office of Child Care after calling another agency to report a staff member for taking photos of children in the bathroom and kissing children on the mouth. Facility work with another agency and local law enforcement on an investigation and states they did not know they needed to report the incidents to the OCC
The Office of Child Care received information pertaining to a staff member taking photos of children in the bathroom and kissing children on the mouth. Facility has since terminated the staff member and reported the incident to another agency, however the facility did not report the incident to the Office of Child Care in a timely manner.

LS asked staff what they are required to do if they suspect any abuse or neglect and they reported that they are obligated to report to their principal. The principal also reported that she is required to report to another agency and the facilities upper management. Principal states she did not know she was required to contact the Office of Child Care, therefore has not instructed staff to report to the Office of Child Care.
Staff state that they are required to report suspected abuse or neglect to the Principal of the school per the school policy. Facility is reminded that the license for the child care is separate from the school and they must adhere to the child care regulations. Staff must be informed that they are not required to report incidents of suspect abuse or neglect to the director, other staff, or principal of the school. Rather they are required to report any suspicions directly to the OCC and other appropriate agencies themselves.
The Office of Child Care received information pertaining to a staff member taking photos of children in the bathroom and kissing children on the mouth. Facility has since terminated the staff member and reported the incident to another agency. At the time of the inspection Licensing Specialists observed six children unattended in the hallway, and then later four children in the hallway unattended.
At the time of the inspection Licensing Specialist observed six children in the hallway unattended. The facility was being dismissed from a full school assembly. Licensing Specialist informed the staff that children are to be with approved staff at all times. After observing the next preschool classroom, Licensing Specialist observed four children in the hallway playing in their backpacks and getting water with the door to the classroom closed and no staff supervising. Licensing Specialist informed the staff that they needed to step into the hallway to supervise the children.
At the time of the inspection Licensing Specialist observed children unattended in the hallway multiple times, even after informing staff they must remain with children at all times.
Facility was unable to ensure that each staff was providing appropriate care. Facility states they put the staff member on leave and allowed them to come back and then had to terminate them for kissing a child on the mouth.
Licensing Specialist witnessed teachers coffees accessible to children. This regulation was discussed at the inspection conducted May 22, 2026.

Licensing Specialist observed an old license from 2018 hanging and facility was using rooms listed on that license. The most current license is from 2021 that indicates only two rooms have been approved. Facility is asked to submit a change of facility form to add the third room back to their license and reminded that they are always to post the most current version of their license and only use rooms that the Office of Child Care has approved for care.
