LS observed that seven (7) children are required to have health assessments completed. LS observed that seven (7) children are required to have lead screening completed. One (1) child is required to have immunizations.

LS was informed by the provider that the children use areas of the home that have not been approve by OCC. The children sometimes play in the garage and are allowed to wait for a parent in the sitting room on the main floor.
LS observed that harmful products in the bathroom such as cleaning supplies were not stored properly.
