Room 8: School Age is being used as a Preschool room. Please send the Licensing Specialist an OCC 1209 Change in Facilities form requesting to make Room 8 a Preschool room.

Licensing Specialist observed paint in disrepair in: Rooms 1, 3, 4, 5, 6, 7, 8, and in the children's restroom during the inspection. A ceiling panel in the children's restroom has a screw that is hanging. The cabinet under the sink in Room 3 has chipped wood.
The diapering pad in Room 2 has a large rip on it which is preventing it from being properly cleaned.
The staff listed as the Director has not completed the 45 hour infant and toddler training. Corrected during inspection. The Director was enrolled in an approved 45 hour infant and toddler training during the inspection.
An Aide was observed taking children outside of their room to the bathroom and back to their classrooms.
Licensing Specialist observed a crib mattress with a large rip in it. Facility stated the Agent has extra crib mattresses at her house.
Licensing Specialist observed infants in high chairs without the restraints secured. Corrected during inspection. A staff secured the restraints during the inspection.
Licensing Specialist did not observe a staff wash an infants hands nor their own hands and continued to diaper the next infant.
No bottles had dates during the inspection. Facility stated that they were not aware dates were needed on bottles.
LS observed in classroom Rooom #5 and Roon #6 was not accurate. Children were not signed in and the number of children in the classroom did not match the number of children on the attendance log. Classroom staff corrected attendance log at time of inspection. Non compliance marked as corrected at time of inspection.
LS observed more than one staff member that do not have current updated criminal background check results in the file. The facility shall send a letter of correction verifying that all staff records have been checked for criminal background results.
Specialist observed teacher redirect child by grabbing child's arm.
