LS observed one preschool teacher completed 9.5 hours of training for the previous full year. Individual is using 2.5 hours of training completed in 2025 to count towards last years training requirement. Staff member cannot use that training for this year.
LS observed cleaning wipes in an unlocked cabinet on the first shelf. Facility removed cleaning wipes during inspection.

This monitoring inspection recorded no violations or advisories.
The director self reported that during a valentine's day party on 02.14.25, a 4 year old child walked up the stairs and out the front door. It was reported that the child left the classroom at 12:10pm and was outside at 12:11pm. The police contacted the center at 12:22pm to inform the center that the child was across the street. This information was confirmed by video footage. The operator did not ensure the safety and security of the 4 year old child during this time.

LS reviewed emergency cards. Several children were missing the schedule and source of healthcare. Facility is reminded to ensure that all emergency forms are complete.
LS reviewed all staff medical forms. Several medical forms were missing physician contact info stamp or address and phone number of the physician.
LS reviewed training certificates for preschool teachers. Two teachers had less than 12 hours for prior year of training. Those teachers will need to complete additional training to make up for the prior year.
LS reviewed training certificates for aides. One aide had less than 6 hours for prior year of training. The aide will need to complete additional training to make up for the prior year.
LS observed cleaning supplies that were on low shelves making them accessible to children.

A teacher reportedly pulled a child's arm, making him fall and hit his head. The incident occurred at approx. 12:59pm and was brought to the director's attention at 745pm by the child's parents. The facility stated that the teacher did not report the incident to them.
