Licensing specialist observed one school aged child in care who did not have an emergency form. Licensing specialist requested a form for this child and reminded provider that even if they are a last minute drop off and have a sibling enrolled, they still need an emergency form on file.

Licensing specialist observed emergency forms that were outdated and have not been updated by parents.
Licensing specialist observed children between the ages of 18 months- 3 years old sleeping in rockers during nap time, and one three year old sleeping in a play pen. Licensing specialist reminded provider that children must sleep on appropriate furnishings during rest periods.
Licensing specialist noted a cleaning agent in the bathroom that was placed behind the bathroom sink but still accessible to children in care. Provider removed the cleaner.
Specialist observed two children who need lead screenings as soon as possible.
The provider did not submit an updated release for Y.M. within the required time frame.
