LS observed a climber on the playground has a broken crawl through tunnel and a trashcan that has trash in it against the playhouse.
LS reviewed staff medicals one staff member did not have an OCC medical form and 4 had forms that were missing physician information.
LS inspected the outdoor play area for potential hazards. The wood barrier around the mulched area of the playground is at least 7 inches high on the inside and 10 on the outside creating a trip hazard.
LS also observed several nails/screws poking out of the corners of the wood barrier and the rope climbing area of the rock wall is pulled apart.
LS observed a child sized table on the playground that was falling apart. Facility will remove or repair table.
