One emergency form was missing the annual update. The provider is advised to review children's files and ensure that all children's emergency forms are completed and contain the required information. The provider is reminded to maintain emergency information for all children in care. Please ensure that the parent reviews, completes, signs, and dates the emergency form and submit evidence to the Licensing Specialist once corrections have been made to the emergency forms.
