The child safety latch that was located on the cabinet underneath the kitchen sink was broken on the left cabinet. There were cleaning agents located under this sink. Please fix or replace the latch and submit a letter of correction immediately.

There were two children's files that were missing the lead screening forms. Obtain this lead screening form and maintain in the child's file. Please send a copy of the completed form for each child to the Licensing Specialist.
The consumer education pamphlet (Guide to Regulated Child Care) was missing in all of the children's files except for one child. This form will be emailed to the provider today so that she can advise parents of this information. Please email the Licensing Specialist when the parents have been advised.
The fire drill log was missing for 2024. The provider was unable to locate this form. Ensure that the fire drills and the emergency disaster drills are being conducted and logged as required.
The provider stated she did complete the updated 2023 Basic Health and Safety Training, but she did not have a copy of the training, The provider stated she will attempt to obtain a copy of this training or she will take the training again. Submit a copy of the certificate to the Licensing Specialist.
Continued training was submitted late for the 11/2022-11/2023 timeframe. Please submit at least 12 hours of continued training for the 11/2023-11/2024 timeframe by 11/30/2024.
