LS observed two, filing cabinets, stacks of boxes, three copiers, two step stools, many books all over the place, six or more blue chairs, two media carts, sixteen music book stands, and a large television laying up against the bookcase. LS observed a large heater unit in the back of the room pulled apart and the parts pulled from the heating unit laying on the floor and wall. LS observed serval bookcases with missing bottoms with large mouse traps placed on the inside of all the bookcases. LS requested for the group of children and staff to move to the cafeteria. The Teacher stated that they go to the multipurpose space for snacks and supper only. LS observed lose and hanging wires coming from an outlet on the right side of the wall in front of the desks in the media center.

LS observed evidence of the children and staff members located in the medica center. LS observed the elementary school using the media center as storage space. LS observed two filing cabinets, stacks of boxes, three copiers, two step stools, many books all over the place, six or more blue chairs, two media carts, sixteen music book stands, and a large 70-inch television, laying up against the bookcase. LS observed a large heater unit in the back of the room pulled apart and the parts pulled from the heating unit laying on the floor and wall. LS observed serval bookcases with missing bottoms with large mouse traps placed on the inside of all the bookcases. LS requested for the group of children and staff to move to the cafeteria. The staff member stated that there are other groups that meet in the cafeteria. The staff member stated that the Butterfly Program, Basketball, Soccer, Jump Rope Group, Achieve, Soaring Eagles and the Homework Program met in the cafeteria. All the groups are directed by Teachers that work at the elementary school. The staff member stated that the groups have been meeting since the beginning of the school year. LS observed children located in the media center with many hazards. The media center is restricted until the corrections have been made.
