The director will need to repair the floor vent in the infant play area. The bathroom area where the soap dispenser was removed needs to repaired.( exposed nails) .

One Aide has not completed their Aide orientation.

The director will need to purchase two gauze rolls, whistle ,and plastic bag for first aid kit.

Specialist observed two children in care missing the authorized pick up person, at least one emergency contact, physician or source of health care information, and the signature and date of the parent or guardian. Facility is asked to obtain the missing information and submit a letter of corrections upon completion.
Specialist was unable to review a current medical evaluation for one staff member at the time of the inspection. Facility is asked to have the staff member complete the evaluation and submit a copy to the OCC upon completion.
Specialist did not observe a written individualized activity plan for any of the infants in care. Facility is asked to obtain this information and submit a copy to the OCC.
Specialist observed loose fitting sheets in three cribs used for children. Facility is reminded that a tight fitting sheet for each child in the center must be present.
