LS observed a cord plugged into an outlet in the bathroom which was hanging too close to the paper towel dispenser, posing a tripping hazard.. Provider unplugged the cord during the inspection.

LS did not observe evidence of lead testing for a child born on or after January 1, 2015. Provider must immediately submit evidence of lead testing or an appointment with a health care provider to obtain one.
LS did not observe the physician's signature verification on page 2. Provider must immediately submit the corrected form with the physician's signature.
LS did not observe written requests to rest in a crib for two children who are at least 12 months of age and younger than 2 years old. Provider must place children on a cot, mat, or sleeping bag for rest time or immediately submit a request to remain in the crib until age 2 written by the child's parent.
LS observed a soft item in the crib with a resting child.
LS observed that the faces of all resting children were covered with blankets and the cribs were covered with several blankets. Provider corrected during the inspection and removed the coverings.
This mandatory review inspection recorded no violations or advisories.
