LS observed no evidence of qualified staff in the Grasshoppers and Monarchs classroom. Facility was able to obtain qualified staff in each classroom at the time of the inspection. Facility must submit new staffing pattern showing qualified staff in each classroom immediately and submit a memorandum of understanding of the regulation and maintaining qualified staff in each classroom at all times.
LS observed photographs of drug paraphernalia found in the staff restroom that was used by the accused after the Director inspected the space. LS observed photographs of drug paraphernalia found in the staff restroom that was used by the accused after the Director inspected the space. Director explained the accused's behavior after returning from the restroom, i.e; eyes rolling back, nodding off, and slurred speech. Facility to submit evidence of all staff training in regards to this regulation and facility policies in the same regard, as well as how staff training will be implemented moving forward.
