LS came into the daycare area and observed child sleeping in an infant seat. Per the provider the child had just fallen asleep and was immediately removed from the infant seat. Once the child was moved, the child woke up and the provider let the child remain up and held the child who was crying at the time. Corrected while on site.

A follow up visit was conducted and observe 5 three year old children in care. after the site visit of last week , informed children can not return until documentation has been received. Today arrived at site and found 5 children of which there were 2 without medical records. Informed provider to contact parents to pick up children and can come back until all medicals records are received. Parents called and informed she just got to work and unable to pick up children. She just started working and will have to pick child up later. the child can not return until all forms are complete. Due to the lay off and information given to specialist, informed parent child can not come back tomorrow. A follow up visit will be conducted and this will be discussed with the supervisor. ,
Upon arrival , licensing specialist asked provider for emergency forms for children in care. These forms were not available for review. Provider called parent and parent emailed emergency forms. After review by specialist missing 2 children emergency forms. These forms must be submit to office of childcare. Ls discussed with the provider that children records must be accessible and updated on forms supplied by the office of childcare. Corrected while on site.
Ls discussed annual update for emergency forms. Emergency forms were not updated by parents.
Licensing specialist observed child sleeping in an infant seat and discussed with provider to remove child. Child was removed and placed in appropriate sleeping surface with hesitation. Corrected while o site.
