At the time of the inspection the LS conducted a walkthrough and observed open bags of snow salt sitting in the outdoor play space.
At the time of the inspection the LS asked for the attendance, the provider gave attendance for Jan. & Feb. but not March. When the LS asked for the attendance sheet the provider stated she uses the provider portal and text to create the attendance on the 15th of each month.
At the time of the inspection LS reviewed the children files and noted child 1 and child 2 is missing medication EpiPen and alburtol, child 3 is missing part 1 of the health assessment.
At the time of the inspection LS completed a walkthrough of the kitchen, a pot of old oxtails were sitting on top of the stove. The oven also needed cleaning, the cover of the door needed to be fixed.
At the time of the inspection the LS reviewed the first aid kit and provider was missing flashlight, surgical gauze pad, paper towels, scissors, soap, & tape. The provider was able to correct on site all but the flashlight and tape.
At the time of the inspection the LS observed a bag of snow salt in the front yard where the children toys are.
At the time of the inspection Ls reviewed the children files and observed child 1 is missing the lead form.
At the time of the inspection the LS asked for the sign in sheet and the provider stated she didn't have one right now.
At the time of the inspection the LS observed the drain pipe on the outside of the home broke apart and is hanging.
At the time of the inspection the LS observed the provider is using a warped pallet as a step into the home.
At the time of the inspection Ls observed 3 half eaten cupcakes on the kids table with two used paper cups. When LS asked about the items the provider stated her grandson had cupcakes.
At the time of the inspection Ls observed dirty stagnate water inside of the bucket and child's activity table.
