LS observed several children's written activity plans that had not been updated. The provider has sent a email to each infant/toddler parent requesting that they come in and update the activity plan for their child. LS also advised that the teacher should go over the activity plan with each parent.
This mandatory review inspection recorded no violations or advisories.

LS observed several children in care missing enrollment dates and hours of expected attendance, missing name, address and telephone number of child's physician, and individual who is authorized to pick the child up each day.
LS was not notified of two new staff members and no paperwork was given to the office of child care. During the inspection the owner did provide ROI for one staff.

LS observed one toilet seat that was not properly aligned with the toilet. LS observed chipping paint in several classrooms, pealing window tint on one door in a classroom, hanging blinds cords in one classroom.
LS observed one bathroom that did not have running hot water.
LS observed 2 refrigerators that were dirty.
LS observed potentially harmful items (one two by four and inappropriately stacked toys and trash) behind the shed in the outside play area.
LS observed several infant/toddler activity plans that were not reevaluated at least every 3 months.
LS observed two (2) mattresses in one (1) crib. The director removed the extra mattress during the inspection.
LS observed three (3) cribs that were end to side and touching.
LS observed one storage container that needed a safety lock on it. One storage container with a broken shelf.
LS did not observe emergency escape route floor plan in each area and room in the center.
