Licensing specialist observed several containers of diaper creams in drawers that are accessible to the children. Correct immediately.
Licensing specialist observed infant bottles not labeled with the child's name and dated. Correct immediately.
LS conducted a complaint inspection in which the OCC received information that a baby was thrown and at some time during the incident a staff person and a parent were involved in an altercation in the back of the building on the playground while children and staff were present. Ultimately one staff and one parent were involved in the altercation and then subsequently another staff who tried to break up the altercation was hit and injured. At that time children were evacuated from the playground. The individuals moved inside and continued to verbally go back and forth and at this time, the children were moved back outside.
Attendance by groups of children has not and is not being done by each leader of a group and the group that is transported Correct immediately.
Center does not have a defined procedure in place to ensure the whereabouts of children while in care. Correct immediately.
