Child was left unattended in the classroom for 15 minutes before director found him alone. Staff member turned out the lights and left the classroom. Staff member did not notice child was missing until the child's aunt come to pick him up at the end of the day.
A child was given a dog biscuit to try. Staff member had dog biscuit in her hand when child approached and asked if it was a dog biscuit and if they could try it. Staff allowed child to take a bite.
