Licensing Specialist observed a sign in the provider's front yard advertising the child care that did not have the registration number. Provider is asked to add the registration number and submit photographic evidence of corrections.
Licensing Specialist observed a two year old child in care that did not have any forms. Provider stated that the child was just testing out the child care and was planning to start in next several months. Licensing Specialist informed the provider that no matter how long a child is left in care they must have the required forms. Provider had the child's mother pick them up at the time of the inspection. Licensing Specialist found one emergency for that needs an annual update.

Provider stated that three substitutes from last years inspection are no longer employed but had not informed the office of child care. Provider is reminded that within 5 days of any changes they are required to let the Office of Child Care know.

Provider submitted two substitutes to the OCC with obsolete medical forms. Provider is asked to have substitutes re-do medical evaluations on the updated forms.
