Complaint states that eight children of various ages were observed playing in the street of the cul-de-sac. The children were observed being supervised by an African American male who was around 19 years old , and an African American female between 13 and 15 years old.
Complaint states that eight children of various ages were observed playing in the street of the cul-de-sac. LS informed provider that the daycare children should not be playing in the street . The children should be playing in the Provider's backyard.
LS observed that 1 child is missing an emergency and 1 needs to be updated, 1 child is missing part 1 of the health assessment and 2 children are missing part 2 of the health assessment.
LS observed that 3 children are missing lead screening or test.
LS observed that the bathroom used for child care does not have a window or a working vent.
LS observed a substitute that has not been approved by the OCC.
This mandatory review inspection recorded no violations or advisories.
