LS did not observe part 2 of the medical health assessment for one child in care.
When LS arrived a staff that was not on the Personnel List was present, however this staff was known to OCC. Requesting that the provider send an updated personnel list, listing all staff that work at this location.
LS observed 43 children alone in the gym with one (1) staff. The other staff were on the premises however they were not supervising the children in the gym. One (1) staff was in the cafeteria setting up activities. One (1) staff was in the hallway hanging up materials and the Director was in the ladies room at the time of arrival. During the inspection when the children were brought back to the cafeteria all staff remained with the children and staff child ratio was maintained.
