The Center did not have a written agreement, as outlined in COMAR, for the majority of the children in care whose records were reviewed by LS.
Five children in care, whose records were reviewed did not have a lead screening test.
When LS arrive the PS class was in the main parking lot which is an unapproved area. Children were running into the road and parking lot away from the teachers. One of the teachers was on their phone for several minutes.
When LS arrived at the Center the PS class was in front of the facility which is not an approved area. Children were running in the parking lot and road away from the teachers. Which LS was observing one of the teachers was looking down at their phone for several minutes not supervising the children in their care.
