During the review of children's records, no evidence was found to indicate that a four year old boy's parents were given the Consumer Guide to Regulated Child Care. Please provide this information to his parent(s) and have one of them sign that they received the information. Please provide proof of receipt by the parent(s) to OCC for the correction of this non-compliance.

During a review of children's files, no evidence was found of a one year 4 month old girl receiving any blood lead test. One is required at 12 months and at 24 months of age with the tests results being placed in the child's day care file. During the inspection, the Provider contacted the child's mother and informed her of the need for her child to have blood lead testing and asked her to provide the test results or schedule an appointment for testing. Please provide a copy of the blood lead test results to OCC for the correction of this non-compliance.
During a review of emergency forms, it was found that a two year old child is missing her physician's phone number and a 6 month old, sibling of another child in care, is missing her home address on her emergency form. Please have the respective parents add the missing information to their child's emergency form. Please provide a copy of the emergency forms, including the added information, to OCC for the correction of this non-compliance.
