There were 2 notes for children that were injured during care, one for a child where a shelf fell on a child and one where a child fell and "busted" his lip open. Neither one was signed by a parent. One was not available, so the director re-wrote it and asked the parent to send documentation that she was told about the injury. The one for H., who fell outside did not have a form. There was a form for a different child, N.
The changing pads in the infant and the toddler rooms had rips and tears in them. The director had a work order that has changing pads ordered for the infant and toddler rooms on October 15, 2025. There is a spot at the entrance of the boys room that has a piece of the floor missing and needs to be repaired, the adult bathroom is out of order and the staff are using the same bathroom the children are using.
Equipment was being repaired on the playground and had a wet paint sign on it. The steps were being repaired as a child fell and was injured. There was still standing water on the tarp over the play toys on the infant playground. There were several bee's or wasps nests seen under the awnings of the infant and toddler playground door and the 2 year old playground door.
There was a piece of climbing equipment that was being repaired and repainted and was still under repair. It had a wet paint sign on it. The director states that piece of equipment is not being used at this time. Children are being directed around it and they go for walks in the stroller.
The director stated that she saw teachers standing around the fence talking and she went out to tell them they needed to be supervising by circulating the playground. This is when the child fell on the equipment and hurt his lip.
The center had a child seen in a video, that had large bumps and red spots that were not covered on the child's hands, and legs. The child was in care.
The center had 3 confirmed cases of hand, foot and mouth. They did not contact the health department.
There were no handwashing signs posted in the infant and toddler room.
There were no diapering procedures found in the infant and toddler rooms.
The cubbies in the pre-k classroom fell when a child was putting their things away. The cubbies are now secured to the floor and wall to prevent this from happening again.
The cubbies in the pre-k classroom should be secured to the wall or floor to prevent them from falling. The program has corrected this by securing the cubbies to the floor and the wall to prevent them from falling again.
Cubbies fell on a child while another child was putting their things away. The cubbies were not secured to the wall or floor when this happen. Staff assisted and helped the child when this happen. The cubbie are now secured to the floor and can not move.
The cubbies in the pre-k room where not secured to the floor or the wall which cause them to fall when a child was putting their things away. The program has corrected this by securing the cubbies to the floor and wall to prevent this from happening.
LS observed chipping paint on the playground equipment. The playground tarps needs to be replace, wood chips need to be replaced on the playground and the playground needs to be repainted.
