LS did not observe a qualified staff member with the group of children in Multipurpose Room 2. The operator shall assign qualified child care teachers to each group of children as needed to meet the requirements for group size and staffing.
LS observed only two staff members present with 67 children on the date of the inspection that were certified in first aid/CPR. A center with an approved capacity of more than 20 children shall have in attendance, in a ratio of at least one staff member for every 20 children present, staff members who are currently certified in approved first aid and CPR.

LS did not observe an incident report for the incident that occurred on March 12, 2025, where a staff member was observed grabbing the arm of a child. The operator shall record or maintain on file each injury or accident.
LS conducted a complaint investigation on today's date. The complaint alleges that on March 12, 2025, a staff member grabbed a child by the arm to escort her back to a table. LS interviewed staff member # 1, that witnessed the incident, who stated that between 1:55 and 2:10 pm on March 12th, the child referenced in the incident did not want to go to the other side of the cafeteria to get help with her homework. Staff member # 1 stated that she witnessed staff member # 2 come over and yank the child's arm aggressively and pull the child over to the other side of the cafeteria. Staff member # 1 stated that she informed staff member # 2 that you cannot grab a child like that. Staff member # 1 stated that when she asked the child if staff member # 2 had grabbed her, the child responded by saying that that staff member # 2 grabbed her tight. LS interviewed two other staff members who were present on the date of the incident, but neither one witnessed the incident. The center director also called the area manager for the center, who informed her that she had also not reported the incident to Child Protective Services. An employee who has reason to believe that a child has been abused in the center shall report that belief directly to the protective services unit of the local department.

The operator shall ensure the safety and security of each child at all times.
